Create Initial Super Admin

Bootstrap the first `super_admin` account. After this, only authenticated super admins can create more admin users.

Tenant Scopes

Empty tenant scopes are valid for `super_admin`. Non-super-admin accounts should only receive tenant UUIDs they are allowed to operate against.
